Background Check Authorization Form

I authorize Nurturing Nannies to initiate a background check through Yardstik. I understand that I will receive a secure invitation from Yardstik to provide my consent and complete my portion of the background check process directly.*

I understand that all adult household members (age 18 and older) who live in the home and will be present during care are required to complete and submit their own individual Background Check Authorization Forms before Nurturing Nannies can proceed with services.*
